ABSTRACT
Background: Recurrent early pregnancy loss (RPL) with absent fetal heart sound (FHS) poses significant emotional and physiological challenges. Homoeopathic individualised medicine, based on the totality of symptoms and constitutional characteristics, has shown promising results in select cases¹².
Case Summary: We present a 37-year-old woman (F57B) with primary infertility and a history of two pregnancies ending with absent fetal heart sound, accompanied by obesity, early menses, chronic headaches, anxiety, and marked chilly–thirstless constitution. Conventional investigations were normal, including thyroid profile, diabetes screen, and follicular study.
Her case was managed using individualised homoeopathy, including Magnesia carbonica 200C and repeated courses of Tanacetum vulgare 200C³⁴ based on PQRS symptoms (chilly, thirstless, early menses, anticipatory anxiety, shouting after abortion, milk aversion, foul mouth odour, watery stools).
Outcomes: Following sequential prescribing from October 2017 to August 2018, the patient exhibited constitutional improvement, a reduction in headaches, regulated menses, weight loss, and relief from somatic symptoms.
She subsequently conceived spontaneously. A follow-up scan showed a positive pregnancy with normal fetal heart sounds, marking a successful outcome after years of infertility and two prior pregnancy losses.
Conclusion: This case illustrates the potential role of individualised homoeopathic prescribing in managing recurrent pregnancy loss with absent fetal heart sound. Further controlled studies are warranted.

INTRODUCTION
Recurrent pregnancy loss affects 1–3% of couples globally¹. Causes range from genetic, anatomical, endocrine, immune, and unexplained etiologies². In many women, despite normal investigations, pregnancy ends before fetal cardiac activity develops, leading to emotional trauma.
Homoeopathy aims to address the constitutional imbalance, emotional state, and PQRS symptom patterns³. Tanacetum vulgare has been traditionally used for threatened abortion, uterine dysfunction, and habitual miscarriage⁴.
This case demonstrates successful conception and documented fetal heart activity after individualised homoeopathic treatment in a woman with two prior early pregnancy losses.
METHODS
A single-patient observational case report following CARE guidelines.

RESULTS
After 10 months of individualised constitutional treatment:
- Menses regularised
- Anxiety and sadness markedly reduced
- Headaches reduced from 2/week to minimal
- Weight reduced from 77 → 70 kg
- Gastrointestinal symptoms improved
- Constitutional symptoms: Better sleep, reduced fear & anticipatory anxiety
- Confirmed spontaneous conception
- Ultrasound (19/08/2018): Positive fetal heart sound
No adverse events were reported.
DISCUSSION
The patient’s clinical picture—recurrent early losses with absent fetal heart sound—falls under the “unexplained RPL” category. Studies suggest homeopathy can assist by improving systemic and emotional health, thereby supporting implantation and fetal development⁵⁶.
Magnesia carbonica matched the early PQRS: irritability, gastrointestinal disturbance, early menses, milk aversion.
Tanacetum vulgare is historically used in recurrent miscarriage, uterine atony, and emotional after-effects of abortion⁷⁸.
The therapeutic success may be attributed to:
- Individualized remedy selection
- Integration of mental–emotional symptoms
- Repeated constitutional balancing
- Improved general vitality and weight correction
This case adds to the growing evidence supporting homoeopathy in recurrent unexplained pregnancy loss.
CONCLUSION
This case demonstrates that individualised homoeopathic treatment can contribute to successful conception and restoration of fetal cardiac activity in women with recurrent pregnancy losses where conventional investigations show no abnormality.
Further randomised clinical trials are recommended.
